Simply complete the form below. For each
item you would like to order, enter the Quantity and Size
you want.
Upon receipt of your form, we will send
you an invoice itemizing your order and bill you for 25% of
the total. The 25% deposit must be paid by the order
deadline date (October 24, 2008) for your order to be
placed. We will send you a confirmation deposit receipt
email. NO DEPOSIT REFUNDS. NO RETURNS, NO EXCHANGES on any
seasonal special orders.
A 2nd Invoice for the balance due will be
sent to you one week prior to shipment of your order.
Shipment will be made upon receipt of your final payment.
